We are looking to recruit a dedicated and experienced Advanced Clinical Practitioner (ACP) to join our dynamic primary care team. This is a flexible role with options for full or part-time hours, working across all ofour three GP practice sites. The ideal candidate will be confident in managing a range of acute and chronic conditions independently, with a patient-centred approach and a commitment to high-quality care.

Main duties of the job

As an Advanced Clinical Practitioner, you will work autonomously to assess, diagnose, treat, and manage patients with a wide range of acute and chronic conditions. Your role will include delivering high-quality consultations (face-to-face, telephone, and online), prescribing and reviewing medications, and supporting long-term condition management.Home visits and supporting end-of-life care may also form part of your duties.

Statement Of Purpose

Job Purpose

As an Advanced Clinical Practitioner, you will be acting within your professional boundaries, providing care for patients from history taking, clinical assessment, treatment and evaluation of their care under the supervision of a GP. You will demonstrate safe, clinical decision-making and expert care, within the remit of your training for patients within the Practice.

Principal Accountabilities

Clinical

1. Review patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs, diagnose, treat, and refer where necessary.

2. Assess, plan and provide skilled, high quality nursing care for patients with palliative care needs and support the family both prior to and immediately after death. Anticipate and respond to changing patient needs whilst maintaining their independence, privacy and dignity.

3. Participate in preventative healthcare services as delegated and agreed by the Practice.

4. Undertake face-to-face, telephone, and online consultations for routine problems as determined by the practice including management of patients with long-term conditions.

5. Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.

6. Contribute to the delivery of the QOF and Enhanced Services targets to consistently achieve high standards of safe, evidence-based, cost-effective patient care and service delivery.

7. Refer patients directly to other services or agencies using appropriate referral pathways.

8. Utilizing clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks.

9. Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.

10. Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

11. Undertake home visits when required.

12. Request and interpret results of laboratory investigations when necessary.

13. Developing and agreeing a personal development plan (PDP) utilising a reflective approach to practice, operating under appropriate clinical supervision, and participating in continuing professional development (CPD) opportunities by keeping up to date with evidence-based knowledge and competence in all aspects of your role, meeting clinical governance guidelines for continuing professional development (CPD).

Administration

14. Participate in the administrative and professional responsibilities of the practice team.

15. Work closely with other members of the MDT in the setting up and/or improving of systems for monitoring and measuring performance against the PCN DES, QOF and any other relevant targets.

16. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes using agreed read codes.

17. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.

18. Restocking and maintenance of clinical areas and consulting rooms

19. Attend and participate in practice meetings as required.

20. To collect data and participate in regular practice audits and research projects.

21. To contribute to the preparation of any practice development plan

22. While seeking treatments, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.

23.
