Job summary

An opportunity has arisen to join our existing nursing team at Darwin. We currently have a Nurse Associate working with us who is an integral part of the team. She will be able to offer ongoing support to the successful applicant. We are a progressive and multidisciplinary team and are eager to expand our existing workforce with diverse roles.

Main duties of the job

The Nursing Associate will provide a high standard of nursing care to the patients of the Practice; contributing to the ongoing assessment, providing and monitoring care based on evidence. Work in collaboration with a range of other professionals including GPs, practice nurses, district nurses, and other community services

Darwin Medical Practice provides services from Greenwood Health Centre, Lichfield Road, Burntwood, St Chads Health Centre, Dimbles Lane in Lichfield and Chasetown Medical Centre, 29-31 High Street, Chasetown, Burntwood.

Job responsibilities

Job Purpose

To provide a high standard of nursing care to the patients of the Practice; contributing to the ongoing assessment, providing and monitoring care based on evidence. Work in collaboration with a range of other professionals including GPs, practice nurses, district nurses, and other community services

Principal Accountabilities

Clinical

1. Provide compassionate, safe and effective care and support to patients

2. Monitor the condition and health needs of patients within their care on a continual basis

3. In partnership with patients, families and carers, ensuring appropriate escalation to a registered nurse or other professional when required.

4. Support patients to improve and maintain their mental, physical and behavioural health and well-being.

5. Promote comfort and well-being by ensuring that patients personal and social needs are met and be able to care for a patients nursing needs.

6. To safely undertake the following procedures to meet the patients nursing needs to include; undertaking routine procedures including venepuncture, ECG recording, recording monitoring , vital signs, wound care, nutrition, hydration, foot care, skin integrity.

7. Carry out immunisations in accordance with local and national PGDs (once additional training has been completed)

8. Provide general and specific health screening, including cervical cytology (once additional training has been completed) to the practice population (within agreed protocols), referring to the General Practitioner when necessary.

9. Identify and report changes in a patients condition to a registered nurse or other professional.

10. Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members.

11. Work with registered nurses and other professionals and engage with other agencies and community teams to ensure a safe, personal approach for patients.

Administration

12. Participate in the administrative and professional responsibilities of the practice team

13. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes using agreed read codes

14. Restocking and maintenance of clinical areas and consulting rooms

15. Attend and participate in practice meetings as required

16. In the course of seeking treatments, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately

17. In the performance of the duties outlined in this job description, the postholder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential

General

18. To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the practice

19. To maintain own continuous professional development and ensure PDP kept up to date in line with the requirements of the practice

20. Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments

21. The post holder should recognise the importance of effective communication within the team and will strive to:

The Practice is open from 8.00am until 6.30pm, with regular extended hours surgeries. It is expected that all staff will work on a rota basis, according to the needs of the service.
