Job summary

Due to retirement, an exciting opportunity has arisen for an experienced, positive, and proactive Clinical Manager to join our "Outstanding" CQC-rated practice. We are a forward-thinking, well-established SystmOne practice that embraces innovation to improve patient outcomes and efficiency. You will be joining a strong management structure, working alongside our HR Manager, Practice Manager, Estates Manager, and incoming Business Manager, all supported by the Partners and an excellent team of clinical and administrative staff.

Main duties of the job

As Clinical Manager, you will provide clinical leadership, supervision, and mentorshipto our multidisciplinary team. You will have direct line management responsibility for: Clinical Leads Chronic Disease Nurse Specialists Pharmacists Care Coordinators

You will support the designated leads both clinically and managerially who have responsibility for the Clinical Practitioners, Practice Nurses, Nurse Associates, Trainee Nurse Associates, and Healthcare Support Workers.

Your role will include:

  • Managing the delivery of effective and efficient healthcare, including medicines management, chronic disease management, wound care, travel health, child immunisation, and cervical cytology.
  • Providing clinical sessions in an area of interest.
  • Working closely with the Practice Manager to ensure high-quality, cost-effective patient care.

Clinical practice

  • When consulting with patients, the Clinical Manager will ensure they, and the clinical team they manage
  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
  • Proactively identify, diagnose and manage treatment plans for patients
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  • Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including using non-drug-based treatment methods, in line with national and local policies and practice needs
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols
  • Work with patients in order to support adherence to prescribed treatments
  • Ensure clinical practice has a person centre approach with goal based discussions being important within consultations
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
