Job summary

  • The role will provide a specialist Paramedic resource for health care professionals and service users, working in collaboration with other members of the Multidisciplinary Team.

  • To work within the community, as a Paramedic Practitioner, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

  • The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.

  • The post holder must own vehicle for home visits with expensed remunerated in overall financial package.

  • To work within the practices to compile outcome data though-out the 6 month pilot.

Main duties of the job

Assessing, diagnosing and treating patients with consultation and support from the lead GPs.

  • Assess and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team

  • Patients with problems needing referral to secondary care should be discussed with the registered or liaising GP before making such referral.

  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

  • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary

  • Works from the surgery and within communities as an paramedic practitioner caring for patients and families.

  • Works as a paramedic, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.

  • Issue medications as appropriate following policy, patient group directives and local pathways after discussion with lead GP.

  • To formally and informally impart knowledge and skills to colleagues promoting peer review and best practice within the work environment.

  • To communicate at all levels within the team ensuring an effective service is delivered.

  • To maintain accurate, contemporaneous healthcare records appropriate to the consultation.

About us

We are an established training practice comprising of two surgeries. We are providing medical services to a list of 13700 patients across the two sites, with a multi-disciplinary team of GPs, Paramedic, PNs, Pharmacist and Pharmacy Technicians, HCAs, Health and Well-Being team and a supportive non-clinical team.

Qualifications

Essential

  • Hold a BSc in a training programme approved by the College of Paramedics
  • Registration with the Health & Care Professions Council (HCPC)
  • Hold or working towards a non-medical prescribing qualification
  • Hold a full UK driving license.

Desirable

  • Experience of working within Primary Care
  • Completion of the FCP programme

Experience

Essential

  • A minimum of 3 years experience working as a paramedic (in Primary or Secondary care) or a band equivalent role.
  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
  • Demonstrates accountability for delivering professional expertise and direct service provision
  • Exercise a critical understanding of personal scope of practice and to identify when a patient needs referring on
  • Through patient assessment and working in partnership with patients and their carers, make decisions about the best pathways of care, informed by the urgency and severity of patient need, patient acuity and dependency, and the most appropriate deployment of resources.
  • Manage risk in unpredictable, uncertain situations to uphold patient safety, including by referring on to other primary care team members and to specialist services
  • Able to follow legal, ethical, professional and organisational policies / procedures and codes of conduct.
  • Involves patients in decision making and supporting adherence as per NICE guidance.
  • Able to write comprehensive clinical notes and use clinical coding
