Job summary
We are seeking an exceptional practice nurse to deliver an assessment, treatment, screening and advice service to our patients. You will develop and manage care programmes for individual patients within specific groups and facilitate health promotion, chronic disease management and clinical audit.
The successful candidate will be a registered nurse with prior experience of working in a GP practice. You will have knowledge of public health issues and experience of chronic disease management.
This role is available ideally for 30 hours a week over 4 days (with either a Tuesday or a Wednesday off). The applicant must be available to run one clinic per week during extended hours, either before 8.00am (Monday/Tuesday) or after 6.30pm (Wednesday). The rate of pay will be discussed at interview and is dependent on prior experience and relevant qualifications, in line with recommended guidance on GPN pay, and terms and conditions. We offer 25 days annual leave plus eight bank holidays per year, pro rata.

Main duties of the job
- provide a nurse-led service for Brockwell Park Surgery patients, delivering appropriate assessment, treatment, screening and advice.
- develop and manage care programmes for individual patients within specific groups.
- be responsible for a wide range of routine treatments and tasks such as health promotion, chronic disease management, immunisations, cytology, wound care, removal of stiches, venepuncture, health prevention, NHS health checks.
- participate in quality and audit initiatives, ensuring that evidence-based nursing care is delivered to the highest possible standard.
- act as a resource for members of the team and other health professionals, ensuring that relevant information is shared effectively.
- support the management team in the reviewing of clinical policies and procedures.
- work at all times within the NMC Code of Conduct.
About us
Brockwell Park Surgery is a friendly training practice located close to Herne Hill station. The surgery has a strong, longstanding ethos to provide first-rate patient care.
We pride ourselves on delivering excellent access to GP appointments. We are rated good by the CQC in all domains and continually strive to provide the very best service to our patient population.
The practice is led by an enthusiastic and proactive team of three GP partners who are involved on a day-to-day basis with the running of the practice, each taking a role in management and supported by a practice manager. In addition there are currently seven salaried GPs, trainee doctors, two practice nurses, a pharmacist, physiotherapists, social prescribers, and a team of receptionists and administrators.
We are particularly proud of our beautiful garden which is a haven for our staff and patients. The practice runs a garden therapy project, to support the wellbeing of patients.
