Health and Wellbeing Coaches are an effective intervention to empower people to develop their own knowledge, skills, and confidence to self-manage their healthcare.

Main duties of the job

As our service expands, we have vacancies in our Social Prescribing team for Health & Wellbeing coaches (HWCs) to work across our North Somerset, Bristol, and South Gloucester sites. HWCs use coaching skills to help individuals to identify, set and track personalised health and wellbeing goals and will provide support on a 1-1 basis or via group coaching sessions. To empower patients to take control of their health and wellbeing with health improvement changes such as weight loss, eating well, increasing physical activity levels, and stopping smoking. Providing support to adults by telephone, video call or face to face in community venues on a 1-1 and group basis.

The Health and Wellbeing Coach will develop good working relationships with:

Patients

Clinicians and the wider surgery staff

Service providers, 3rd sector and other groups.

Key Responsibilities:

  • Provide information and advice to clients on a range of health improvement subjects in particular weight loss, eating well (and drinking less alcohol), increasing physical activity levels, and stopping smoking.
  • Make recommendations for lifestyle changes, set timescales for implementation and to follow up actions set with clients and support workers where applicable.
  • To plan and facilitate the delivery of groups and workshops focussed on weight loss, healthy eating and physical activity.
  • Record and evaluate a caseload of client contacts and progress on a weekly basis ensuring all information is accurately recorded in line with procedures and guidelines used by the service.
  • Communicate with colleagues and partner agencies to establish referral routes into the service, in particular, from groups with poorer health outcomes, those experiencing health inequalities, including from areas in BNSSG with the highest levels of multiple deprivation.

  • Experience of managing a caseload of clients experiencing health inequalities and delivering healthy lifestyle advice on a 1-1 and/or group basis, translating health improvement knowledge into practice.
  • Ability to record and evaluate outcomes of 1-1 and group support and recommend solutions to ensure best outcomes for service users.
  • Ability to write up case study reports and good news stories.
  • Ability to use a case management system and variety of IT systems competently.

Desirable

  • Experience of planning and delivery of health improvement content in 1-1, group and workshop settings.
  • Knowledge of models of behaviour change to increase patient activation to make positive changes.

Qualifications

  • NVQ level 3 qualification relating to public health, health improvement, physical activity, nutrition or behaviour change.
  • Evidence of continued learning/personal development in relevant field
