Registered Nurse (NMC)

Experience in primary care is desirable, but training will be provided for the right candidate

Strong communication and team-working skills

Main Duties

Provide general nursing services including chronic disease management, immunisations, wound care, and health promotion

Work collaboratively with GPs, nurse practitioners, and HCAs to provide holistic patient care

Manage QOF and long-term condition targets

Key Responsibilities

Clinical practice

Provide assessment, screening treatment, services and health education advice

Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears, etc.

Assist with the organisation and co-organisation of the provision of nursing services for the practice

Provide general nursing treatment to patients in participation with general practitioners using agreed protocols

Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition to include health promotion and well being

Be competent and confident in managing people with long term health conditions

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate

Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug based treatment methods using a management plan, and in line with national and local policies and practice needs

Work with patients in order to support compliance with and adherence to prescribed treatments

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care

Support and manage health needs of women presenting for family planning and cervical cytology consultations

Implement and participate competently in vaccination and immunisation programmes for both adults and children

Advise, support and administer vaccinations competently where appropriate for patient travelling abroad

Promote and deliver evidence-based care for patients presenting with aural conditions

Assessing and treat routine wounds including trauma

Assessing to support good wound management in complex situations (e.g. use of Doppler technique)

Assisting the general practitioner and/or undertaking some simple procedures

Maintain the stock

Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)

Act as a chaperone when required following the practice chaperone policy
