Job summary

Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening, Furthermore, they embed population health management approaches within general practice, targeting health inequalities and improving access.

The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

This organisation must ensure that the post-holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis. Main duties of the job

  • Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
  • Support in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives
  • Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice
  • Embed population health management approaches within general practice, targeting health inequalities and improving access.
  • Teach and advise patients and their families on how to manage their condition
  • Participate in clinical audits and research projects and implements changes as required.
  • Work within national and local protocols where these exist
  • Recognise the boundaries of their practice and know when and to whom patients should be referred
  • Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
  • Participate in the management of patient complaints and learning brought about through incidents and near-miss events
  • Lead on public health campaigns (e.g., flu clinics)
  • Provide wound care (ulcer/Doppler, etc.) to patients
  • Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
  • Maintain chronic disease registers and implement an effective call/recall system.

We are based in modern premises, purpose built approximately 13 years ago, serving a growing list size of approximately 10,300 patients, covering the town and semi-rural environ of Newton Abbot, and are rated Good by the CQC.

Albany Surgery is a member of the Newton West Primary Care Network, a thriving PCN serving 35,000 patients. We work closely with Kingskerswell and Ipplepen and Bovey Tracey and Chudleigh practices, the other practices within our PCN. This collaborative approach is supported by a PCN support team, with a lead manager, social prescribers and health and wellbeing coaches working across the member practices.

As a practice we are committed to staying at the fore front of changes within General Practice, as such we recognise that the support, growth and training of all levels of our staff, both clinical and administrative, is an essential part of the current and future NHS. Education and learning are our core passions and we have 3 GP trainers hosting a number of GP registrars, we support medical student training placements of all years, and mentor health care professionals undertaking Prescribing qualifications and Advanced Practice postgraduate training. The following are the core responsibilities of the Practice Nurse in delivering health services:

  • Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
  • Lead on public health campaigns (e.g., flu clinics)
  • Provide travel advice services
  • Provide wound care (ulcer/Doppler, etc.) to patients
  • Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
  • Contribute to practice targets (QOF, etc.), complying with relevant guidance
  • Maintain chronic disease registers and implement an effective call/recall system
  • Maintain a clean, tidy, effective working area at all times
  • Chaperone patients where necessary.
  • Support the team in dealing with clinical emergencies
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels
