Job summary

Are you looking for a new opportunity? Following the retirement of our Advanced Nurse Practitioner, a position is now available for an ANP to join our friendly and approachable team of healthcare professionals.

ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results.

The team are looking for a forwarding thinking ANP with an attention to detail and an ability to work independently within their professional boundaries

Main duties of the job

You will be an autonomous practitioner with sound knowledge of long term and minor illness conditions, working independently to provide urgent same day care and treatment to patients. You will work alongside other health care professionals with support when needed.

The ANP will be competent in practice using their expert clinical knowledge and skills, independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. They will have the freedom and authority to act and make unilateral decisions in the assessment and diagnosis of patients.

Overview

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the General Practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirement for advanced practice must be met.

The ANP will report directly to the Practice Manager or Partners for any clinical queries to assist in the care of Practice patients to include diagnosis, treatment, preventative care, screening and patient education. The post will also include some administrative duties.

Job responsibilities:

To act as an autonomous allied healthcare professional within allocated surgery clinics to assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Work with patients in order to support compliance with and adherence to prescribed treatments

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Support and manage health needs of women, family planning, cervical cytology or sexual health consultation

Support and participate in shared learning within the organisation

Health promotion acquire data and act upon any areas of concern by giving advice and / or literature or referral to a doctor to promote health and wellbeing of the patients, including:

Smoking cessation advice

Chronic disease / illness management i.e. CHD, CVD, Spirometry, Mental Health, Epilepsy

Hypertension monitoring and blood pressure recordings as per Practice / National guidelines (NICE)

Recording and downloading of ECG in emergency situations

Assist with dressing and removals of sutures when assistance is required by the Nurse team

Attend to patients in need of wound assessment with assistance from Practice Nurse/ District Nurse / Doctor where required

Influenza vaccinating and any other seasonal vaccines (training will be provided)

First aid and assisting in management of medical emergencies

Urinalysis

Provide lifestyle advice to patients making any necessary referrals within the Practice

Weight monitoring

Application of health and safety and infection control

To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level
