Job summary
We are currently looking for an experienced, enthusiastic, flexible and reliable Primary Care Practice Nurse to join our friendly and supportive team.We are looking for a Nurse who has previous experience of working in primary care as a Practice Nurse and unfortunately, we are unable to accept any applications who do not have this experience.
Hours:We are looking for 21 hours a week but will consider up to 37.5 hours a week, within the Practice opening times of 07.30-18:30 (Monday-Friday) with enhanced hours on a Thursday evening until 8:00pm.

An exciting opportunity has arisen for an experienced Practice Nurse to join our established and collaborative Nursing team at our forward thinking and supportive practice. We have a dynamic team of nurses, healthcare assistants and phlebotomist providing chronic disease management and enhanced services to our 14,500 patient list.
We are looking for:
- Proven experience in a Primary Care setting
- Competence in chronic disease management, specifically in Diabetes or Asthma (minimum requirement)
- A commitment to ongoing professional development and willingness to undertake further training as needed
- A team player who is proactive and adaptable to the evolving challenges or Primary Care

Main duties of the job
To provide and maintain a high standard of nursing care for patients, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
The duties will include but not limited to day to day primary care nursing such as phlebotomy, ECGs, wound care, adult and child immunisations, B12 injections, BP monitoring as well as Enhanced Practice Nurse skills in chronic disease management, cervical screening and compression dressings.

Job responsibilities
Primary:
Able to document contemporaneous Consultations according to NMC guidelines using EMIS clinical system
Maintain accurate clinical records in conjunction with legislation
Ensures awareness of statutory and local clinical protection procedures, including systems of referral.
Provide routine nursing care to patients as required in accordance with clinical based evidence
Perform venepuncture according to local guidelines
Request pathology services as necessary

Liaise with external services / agencies to ensure the patient is supported appropriately
