We are looking for a dedicated and organized individual to join our team as a Care Navigator/Administrator. The successful candidate will play a crucial role in ensuring the smooth day-to-day operations of our office.
At the MacMillan Surgery, we understand the importance of building strong connections among our team members. We organize two team bonding events annually to create a fun and supportive work environment outside of regular office hours.
Main duties of the job
The main duties for a Care Navigator/Administration will include:
- Face to face and telephone interactions with patients to deal with enquiries such as: booking appointments, blood results, referral, medication queries etc.
- Liaising with community services such as: district nurses, secondary care, community matron, ambulance services, pharmacies, etc.
- You will be required to maintain confidentiality at all times.
- Good communications skills both written and verbal.
Ideally, experience with Word, EMIS and Docman 10.
About us
We are a practice based in the St. Chads Centre, which is located in centre of Kirkby.
The practice is located in a purpose built Health Centre which
hasaccess to x-ray, midwives and community bases
clinics such as respiratory, diabetes and cardiology. The clinical team
consists of 1 full time senior partner, 4 part time salaried GPs, 2 Practice
nurses, 1 HCA, 1 pharmacy technician, 1 clinical pharmacist and 1 advanced nurse practitioner. We have a professional and experienced administration team with 1 office manage. Our practice list size is 7100
(approx.) and we use the EMIS clinical system, Docman 10, AccuRx and PATCHS.

The following are the core responsibilities of the care navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.
- Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting conditions
- Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately
- Process patient requests for appointments
- Process repeat prescription requests
- Initiate contact with and respond to requests from patients, team members and external agencies
- Enter read/SNOMED CT code data on EMIS and Docman 10
- Photocopy documentation as required
- Data entry of new and temporary registrations and relevant patient information as required
- Input data into the patients healthcare records as necessary
- Direct requests for information, e.g., SAR, insurance/solicitors letters and DVLA forms, to the administrative team
- Manage all queries as necessary in an efficient manner
- Carry out system searches as requested
- Maintain a clean, tidy, effective working area at all times
- Monitor and maintain the reception area and noticeboards
- Support all clinical staff with general tasks as requested
- Undertake all mandatory training and induction programmes
- Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
