Job summary

The Boulevard Medical Practice is looking to recruit an enthusiastic Practice Nurse, to join our current Healthcare team on a 12 month Fixed Term Contract for Maternity cover.

We can offer a dynamic and innovative working environment, with plenty of development opportunities.

We are flexible with the contracted hours but would be looking for a minimum of 20 hours.

Main duties of the job

The ideal candidate will have experience of working in Primary Care and have qualifications in chronic disease management, cytology and immunisations.

The role of the Practice Nurse in General Practice is to support the Practice Team in providing safe and effective care for patients.

They work alongside the Nursing Team, GPs, Advanced Practitioners and Pharmacists to provide a wide range of care, from taking patient observations, to delivering vaccinations and providing health promotion advice.

They are also involved in the assessment, screening and management of long-term conditions, caring for patients with acute and chronic illnesses and providing health education. Practice Nurses provide person-centred care, taking into account the individual needs and preferences of each patient. They also work to promote health and wellbeing, enabling patients to make informed decisions about their own health and wellbeing.

About us

We are looking for a highly motivated Practice Nurse with good interpersonal and communication skills to join our busy and friendly team with a patient population of around 17,000 patients, for the next 12 months.

We are a forward-looking practice which offers high quality care.

The clinical system we currently use is Systm1.

Supported by the clinical team you will help deliver care within the boundaries of your role, focusing upon supporting patients to be healthy, administering vaccinations, childhood immunisation's, monitoring of long term conditions- including Diabetes, Asthma & COPD, health promotion and screening activities (including Cervical Screening).

The position is available on a part or full time basis (minimum of 20 hours). Our nursing rota is based on a 2 week pattern and hours will be discussed with candidates, but these must meet the needs of the Practice, they will include one late night per week (finishing at 6pm), and occasional Saturday morning clinics (9am-1pm).

Salary is negotiable and dependent on experience.

The Practice Nurse works alongside a team within General Practice and within the Central Halifax Primary Care Network (PCN) The post holder will be an integral part of the General Practice Team.

Key Responsibilities

The Practice Nurse is to have the following key responsibilities and attributes, in delivering health services:

  • Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks under the direction of a registered nurse, with a focus on promoting good health and independence.
  • To provide high-quality holistic and person-centred care to individuals
  • To provide routine care to patients as required in accordance with clinical based evidence, NICE Guidelines etc
  • Work as part of larger nursing team to support colleagues
  • Improve safety and quality of care at every opportunity
  • Work with Practice Nurse Lead to take responsibility for developing own clinical competence, leadership, and reflective practice skills within the workplace
  • Provide support and supervision to other colleagues within the nursing team; Healthcare Assistants, Phlebotomists, Apprentices etc, and those on learning assignments/placements as required.
  • Maintain accurate and contemporaneous patient health records ensuring that clinical data is appropriately recorded with SNOMED codes
  • To conduct Long Term Condition Reviews- such as Diabetes, COPD & Asthma
  • To provide wound care, dressings and other clinical tasks as required
  • Perform and record clinical observations such as blood pressure, temperature, and pulse etc.

  • To provide support during minor operations as required

  • Support delivery of QOF, incentive schemes, quality improvement projects, audits and other quality or cost effectiveness initiatives enhancing service delivery and patient care
  • Assist with COVID-19 tasks as required
  • Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
  • Duties may vary from time to time without changing the general character of the post or the level of responsibility.
  • To have a good understanding of practice protocols and policies
