We are looking for an enthusiastic, well motivated, practice nurse to join our busy GP Surgery. The position is permanent and full time. Riverside Surgery is a busy rural practice and the successful candidate will be a capable and confident team player who values excellent patient care. We are a five site practice so multi site working would be essential. We are looking for an experienced practice nurse with qualifications in Chronic Disease Management.
Main duties of the job
You will have a number of years experience and be able to cover most if not all aspects of Practice Nursing which should include Cytology, Vaccination & Immunisation, Diabetes and/or COPD . We are happy for the right candidate to have specialist training in one subject or another, we will also support you to update your skills.
The right attitude to work and learning is as important as any experience is.
About us
The main site at Riverside surgery is a purpose built practice in the centre of Brigg with four other sites in surrounding areas. Five GP partners care for nearly 25,000 patients; we are a Training Practice, host medical students and nurses and also offer work placements. We are a dispensing practice with a pharmacy attached at two sites. The partners have specialist interests in Cardiology, Dermatology, Diabetes, Minor Surgery, and Substance Misuse. We employ an ANP with an interest in; Paediatrics. The Urgent Care Team manages on the day demand offering emergency appointments and telephone advice. Practice nurses manage chronic disease within the surgery and are supported by seven HCAs. Two Practice Pharmacist's manage prescribing and medicines optimisation. The clinical team is supported by a multi-professional administration team.

To undertake a range of nursing
assessments and provide appropriate care/treatment in conjunction with GPs
according to practice policy, protocols etc. To strive to improve standards of
care and be involved in identifying practice population needs. Chronic Disease
management is an essential part of the role as well as the mentoring of our
Health Care Assistants. Knowledge of the GMS contract, e.g. The Quality and
Outcomes Framework, is a vital element of the post.
DUTIES AND RESPONSIBILITIES
Professional Responsibilities
- To
assist GP's to help deliver effective implementation of management of patients
with chronic diseases as detailed by the Quality and Outcomes framework.
- To
support the implementation of evidence-based guidelines in conjunction with the
practice nursing team.
- To
encourage the development of integrated services within Primary Care to include
integrated practice-based teams and multidisciplinary working.
- To
promote your own continuing professional development.
- To
recognise the opportunities for working in partnership with the Primary Health Care
Team.
- To
participate in an annual appraisal.
Clinical
- To support the practice in achieving quality indicators and targets of the GMS
contract and any relevant National Service Frameworks.
- To
have experience in Chronic Disease Management or willingness to undertake
training.
- To
undertake opportunistic screening when clinically required.
- To
undertake wound care/management/dressing techniques.
- To
perform cervical screening and promote breast awareness.
- To
provide advice on lifestyle issues, assist and support patients in changing
their unhealthy behaviours and refer appropriately.
- To
provide an adult and childhood immunisation service working to Patient Group
Direction
- To
deliver travel health care, including vaccinations and advice for patients -
following appropriate training if required.
- To
recognise the signs of child abuse, family violence, vulnerable adults,
substance abuse, addictive behaviours, suicidal tendencies and be aware of
local, referral procedures.

ensure that current evidence-based approaches to patient are standard practice.
- To
apply infection control measures when collecting and handling of laboratory
specimens, disposing of waste materials, dealing with blood and body fluids
