Job summary

This is a exciting opportunity for a experienced Clinical Pharmacist with excellent interpersonal skills to join a friendly team in Walsall.

Main duties of the job

  • Being responsible for the care management of patients and undertake structured medication reviews
  • Reviewing and actioning clinical correspondence from secondary care
  • Prevention and Tackling Health Inequalities
  • Support the leadership team on ensuring person-centred medicines optimisation and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced services
  • Minor Ailments/See and Treat: depending on competency
  • Support patients to take their medications to get the best from them, reduce waste and promote self-care where possible
  • Being part of a professional clinical network and have access to appropriate clinical supervision
  • Review/action lab and scan results depending on competency.

    St Peters Surgery have already successfully integrated a successful PCN pharmacy team into the way we operate and are now looking to expand on a these roles with the addition of an experienced pharmacist to work alongside our clinical team under the supervision of 5 GP partners.

    Among the regular see and treat/minor ailment clinics, structured medication reviews and repeat prescribing, we are keen to develop the role to better assist the multi disciplinary team in reviewing and actioning clinical correspondence from secondary care.

    • - Must be a qualified Pharmacist

    • - Qualified independent prescriber
