Job summary

A Care Coordinator plays an important part within the surgery, They need to have a high level of administrative experience, they also need to be caring, dedicated, and reliable and enjoy working with a wide range of people. They will participate fully in the running of the Practice within the scope of specified administration duties detailed in the job description. You will need to provide an efficient service for the clinical staff and Patients. Being responsible to the Practice Manager and conducting oneself in a pleasant and business-like manner. It the successful individual with be involved in exciting new income generated projects and support other administrative role within the practice.

Main duties of the job

Manage & Support annual leave and arrange cover where necessary

Manage staff sick leave and arrange cover where necessary

Oversee the day-to-day operation during working hours of the reception team to ensure that

staff achieve their primary responsibilities, and ensuring that the wait on the practice

telephone lines is not excessive at any one time.

Support and monitor staff in the use of IT; organise, oversee and evaluate IT training where

appropriate.

Maintain a good knowledge of the clinical systems and provide training where required to staff

and patients to enhance patient care.

Coordinate and support management of the daily distributions of blood results.

Coordinate and support management of 111 & OOH reports

Coordinate and support management of the Breast Screening DNA follow ups.

Oversee QOF and generate reports on health inequalities.

Oversee Minor Surgery Contract

Manage Weight Management

Manage 3LTC Project & Hypertension Project

Oversee Learning Disability reviews

Manage Care Home Contract

Manage UCPs
