Job summary

Air Balloon Surgery have an exciting opportunity for an independent practitioner with experience of fitting and removal of LARC intra-uterine coils and subdermal contraceptive implants.

Our LARC service provision is currently managed by two of our GPs and we are aiming to expand our service provision to offer additional appointments for fitting of LARC intra-uterine coils and subdermal contraceptive implants over one 3.5-4 hour session per month, including pre-fit assessment and counselling and post-fit advice.

Main duties of the job

The post is an integral part of the multidisciplinary team, responsible for the delivery of highly technical and specialised nursing skills which will include but not be limited to

Assessment and history taking

Clinical examination, including bimanual

Instigation and interpretation of diagnostic investigations

Identifying appropriate methods of contraception

initiation of treatment and plans of care, including independent assessment for provision of medications

Provision of all methods of LARC following discussion and assessment ensuring patient can make an informed choice and consent

Appropriate onward referral to other health and social care professionals if appropriate

Job Purpose

The post holder will be able to understand complex care presentations and establish effective care pathways to address any needs in addition to their LARC requirements. This will include those who need additional consideration due to their complex social, emotional, psychological, and physical needs.

The post holder will provide a high level of personal care to patients to ensure they are treated with dignity and respect at all times, are provided with high quality care, clear and understandable information in a format/language they understand and feel their individual needs have been respected and met. The post holder is accountable for their own practice without direct supervision and will be a direct provider of higher-level nursing practice to patients with contraceptive needs.

Analytical Data and Information

Analyse and interpret physical signs and presenting symptoms in relation to investigative findings to support a pathway of care for the client

Analyse and interpret results and discuss findings with the client offering information, support and referral where necessary

Analysis of clinical records identifying any issues of concern to senior clinical team or provide treat-ment within scope of practice/competence.

Planning and Organisation of Work

This is an independent practitioner post

The patient contact is generally on a one to one basis, with those patients whose birth gender is female

Able to plan own workload and work independently

Carry out nursing duties within Practice Policies and protocols, service procedures and guidelines according to competency and provide feedback on issues of concern to LARC Lead GP

Able to plan work for specialist services and be able to plan work and implement care pathways as delegated by the clinical team

Patient Client Care

Able to work within the requirements of the Bristol City Council GP Sexual Health specification;

Responsibility, for working with medical and nursing colleagues, for the delivery of comprehensive LARC services for the practice population

Follow Practice guidelines on confidentiality; adhere to the law regarding sexual health advice to those under sixteen years; Follow organisational policy and good practice assessment regarding safeguarding young people and vulnerable adults, following appropriate Practice Safeguarding policies

Fitting and removal of contraceptive subdermal implants in line with practice protocol and with overall supervision provided by the LARC Lead GP

Fitting and removal of contraceptive IUD/IUCDs in line with practice protocol and with overall supervision provided by the LARC Lead GP

Carry out Cervical Cytology screening

Counselling and Issuing of Emergency contraception under prescribing supervision of GP.

Provide a comprehensive discussion in the event of an unplanned pregnancy and refer to appropriate agencies as necessary

Perform a holistic assessment of patients attending for cervical cytology smear tests.

Deliver care to patients pertaining to their sexual health needs in line with current FSRH guidance

Assess, advise, prescribe, administer and carry out the full range of LARC procedures required by patients, following discussion with the individual as to their contraception choices, in line with FSRH current guidance.

Satisfy FSRH requirements for five yearly LoC re-certification and updating professional practice and reaccreditation

Carry out competent clinical examination of female patients, including cervical and bimanual examination

Be able to diagnose and clinically manage presentations of sexually transmitted infections and related conditions, and referral to appropriate Sexual Health Services providing patients with appropriate advice. Recognise complex issues relating to patients sexual health needs and seek appropriate advice where necessary.

Ensure knowledge of sexual health, in particular contraception is up to date and follow National recommendations and guidelines for their management. Have an understanding of HIV and its im-pact on sexual health care and contraception choices

To be competent in the management of under 16-year olds and vulnerable adults, working within Fraser Guidelines and liaising with the multi-disciplinary team and safeguarding team as appropriate

If currently not a NMP then the use of PGDs is essential

Maintain accurate contemporaneous electronic patient records, in line with local and national in-formation governance policy/legislation

Interpret results accurately and discuss with patients and associated healthcare professionals (HCP) as required. This may include providing advice to HCP outside the service. Manage results in a timely manner, in accordance with practice policy

Always communicate with users of the service in a sensitive, non-judgemental and supportive manner. Subject matter may be complex and emotive, which may involve breaking bad news.
