Job summary

You will support the practice with delivering care to patients in line with the General Medical Services Contract. You will be consulting with patients using a variety of communication methods including telephone, video, and face to face consultations.

There will be opportunities for you to utilise your skills to improve the health of our patients which could include managing long- term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications and undertaking medication reviews.

You will be expected to deliver safe and effective use of medicines by applying your clinical, pharmaceutical and technical skills to a variety of tasks within the medicines optimisation agenda.

You will also be well placed to contribute to multidisciplinary discussions and ways of working.

We are looking for someone who is enthusiastic, caring, innovative and will play an active role within our team.

Main duties of the job

The successful candidate will carry out structured medication reviews (SMRs) with direct patient contact, and may include contributing to:

- Management of long term conditions

- Minor Illness

- Management of medicines on transfer of care

- Reviewing systems for safe prescribing

- Contributing to repeat prescription authorisations and reauthorisation

- Actioning acute prescription requests

- Contributing to achievement of QOF

- Undertaking clinical audits where required
