An exciting opportunity has arisen to join our dedicated Frailty Team as a Community Healthcare Assistant.
This is a rewarding and varied role where you will play a key part in supporting patients to remain safe, healthy, and independent within their own homes and local care homes. You will be confident working autonomously in the community while also contributing as part of a supportive multidisciplinary team.

Main duties of the job
Key Responsibilities:
- Conducting long-term condition reviews
- Taking and accurately recording patient observations
- Promoting health education and wellbeing initiatives
- Liaising with internal and external multi-disciplinary teams
- Providing administrative support to the frailty team
