As an Advanced Nurse Practitioner, you will be an integral part of our clinical team, working autonomously and collaboratively to provide exceptional care to our diverse patient population.

This role is ideal for an experienced ANP who thrives in a team-oriented environment and is confident in managing a range of clinical presentations independently.

Experience of working in Primary Care and a prescribing qualification is essential for this role.

Hours: 30 - 37.5hrs per week

Main duties of the job

The Advanced Nurse Practitioner will be expected to undertake all duties and responsibilities associated with an Advanced Nurse Practitioner working within Primary Care, including (but not limited to):

  • Work autonomously as an advanced practitioner with an extended scope of practice, contributing to the delivery of comprehensive Primary Care services.

  • Conduct face-to-face consultations, telephone assessments, and occasional home visits, providing accessible and responsive care to our patients.

  • Perform clinical assessments, take patient histories, formulate diagnoses, and develop evidence-based treatment and management plans.

  • Prescribe independently within the scope of professional practice, ensuring safe and effective patient care.

  • Maintain accurate, detailed, and contemporaneous clinical records that clearly document assessment, decision-making, and interventions.

  • Collaborate closely with our multi-disciplinary team, sharing clinical expertise and contributing to a culture of continuous learning and excellence.

  • Ensure all clinical practice aligns with the latest evidence-based guidelines, supporting a modern, prudent, and patient-centred approach to Primary Care.

Clinical responsibilities

  • Diagnosing and treating patients presenting with minor illness.
  • Ensuring clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
  • Provide first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of advanced skills in history taking, physical examination, problem solving and clinical decision making to establish a diagnosis and management plan.
  • Instigate necessary invasive and non-invasive diagnostic tests and investigations and interpret findings and action within own competence.
  • Prescribe and review medication for therapeutic effectiveness, in a safe, effective and appropriate manner as defined by current legislative frameworks and local guidelines, and within scope of practice.
  • Provide safe, evidence-based individualised care to the practice population.
  • Refer patients directly to other services/agencies as appropriate.
  • Work with the multi-disciplinary team within the practice and contribute to practice targets (local and national) with disease management agenda. E.g. QoF.
  • Undertake scheduled home visits for patients unable to attend the surgery as agreed with the Duty GP and within scope of practice.
  • Support other healthcare professionals within the practice and offer advice and guidance within area of competence.
