Urgent Wrap Around Care Nurse

Job summary

This role will be part of a new, innovative city-wide primary care Community Urgent Response Team delivering virtual and face to face clinical capacity to support patients across Cambridgeshire who are acutely unwell and unable to get to their GP Practice. Working as part of a multi professional home visiting team the role will work autonomously using clinical assessment and treatment skills to provide responsive, on the day clinical capacity to patients living in Cambridgeshire.

Our vision is to develop a multi-disciplinary community urgent response service to support some of the most vulnerable patients, providing responsive capacity to our GP Practices and local health and care system. You will be part of a team that supports the Ambulance Service to refer patients who, with support, are able to remain at home. This service will reduce conveyances and enable paramedics to reduce the time spent in a patient home. This will be non planned, reactive work to see patients who have been in contact with the Ambulance Service.

Main duties of the job

  • Triage of patients
  • Catheter care
  • ECG interpretation
  • Wound assessment
  • Wound management
  • Chest auscultation
  • Phlebotomy
  • Cannulation
  • IV antibiotics
  • Supporting junior members of the team
  • Working closely with multi disciplinary team including the local authority

This service is a 7 day service 8-8. Various shifts available

You will be trusted to manage your time effectively, ensuring HCAs receive support and training in a timely way. You will receive support and supervision from our Clinical Director.
