Job summary

We are looking for a Long Term Conditions Nurse to join our team. The role is part time working 22.5 hours per week. The role includes working on site with face to face and telephone consultations, plus the option for some home working. There is some flexibility with the working pattern.

Main duties of the job

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.

The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.

Job description

Job responsibilities

  • To support the delivery of the Ageing Well programme by delivering clinical care to patients and working in partnership with other PCN staff to ensure the program is successful.
  • Assist in and perform routine tasks related to patient care
  • New Patient Medicals recognise health promotion opportunities and refer to clinic/colleagues where appropriate
  • Process data using System One
  • Work closely with a range of internal and external partners to deliver the Ageing Well program.
  • Work with other health care professionals to monitor and manage the treatment of Chronic Diseases.
  • Conduct respiratory, Cardiac and diabetic assessments and reviews.
  • Review medication for therapeutic effectiveness appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols for patients with Chronic Disease.
  • Provide information and advice on prescribed or over the counter medication on medication regimes, side effects and interactions. Prescribe when necessary.
  • Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitate change. Give brief interventional support and refer as appropriate.
  • Work professionally carrying out all duties commensurate with the role and in line with NMC Code of Professional Conduct and the Scope of Professional Practice
  • Keep up to date with mandatory training including safeguarding and data confidentiality etc.
  • Apply infection control measures in the practice according to the practice/local/national guidelines.
