Job summary

we are looking for a Practice Nurse to join our friendly 6 doctor, dispensing practice for 10 hours per week.

Main duties of the job

The successful candidate must be able to work well within a team environment. You must be an experienced Practice Nurse with experience of long term conditions, child immunisations, cytology and travel vaccinations. Preference will be given to candidates with experience or interest in Diabetes.

About us

We have an excellent, dedicated and loyal team working from purpose built premises with a list size of approximately 6100 patients. We use TPP Systm1.

Job description

Job responsibilities

Job Summary

To provide nursing services to patients within the practice area, including treatment, screening and advice within professional competence. To work in accordance with the NMC Guidelines. To undertake administrative work that may be required in order to provide medical services to the practice population, including audits and delivering action plans.

Key Tasks

  • To assist in and perform routine nursing tasks related to patient care, as directed by those to whom accountable, including, (this is not an exhaustive list): immunisations; venepuncture; ECG recording, would care and assessment, including dressings; treatment of minor casualties, caring for the acutely unwell patient including resuscitation in emergencies; cervical cytology, assisting with QOF.
  • To provide health screening to the practice population, including health education and promotion within the scope of experience and qualifications, with referral to GPs or lead nurse, as necessary.

  • To recognise the importance of effective communication within the team,with the wider PHC team and other colleagues, and with patients and carers. The latter should include the recognition of peoples need for alternative style and methods of communication, and respond accordingly.
  • Participate in local meetings as required such as multidisciplinary team and gold standard framework meetings.
  • Liaise with other health care professionals such as district nurses, medical staff and pharmacy technicians, to coordinate patient care.
  • Support junior staff and health care assistants in their role.
  • To maintain the general tidiness and cleanliness of the working environment, with regard to infection prevention and control; maintain supplies and equipment used, as part of the nursing team.
  • To maintain accurate patient records and statistics, participating in search and audits, and summarising and computerising medical notes.
  • To adhere to the practice uniform policy at all times to ensure infection prevention and control risks are minimised.

Key Tasks

  • to manage chronic disease (dependent on training). Monitor, develop and manage individual careplans, agreeing these with the patient and other health care professionals.
  • to keep up to date with current guidance and best practice for the management of these patients, and act as a resource for practice staff, participating in meetings and training as required.
  • to manage patients having warfarin therapy by carrying out INR tests and dosing patients warfarin, ensuring appropriate recording
  • to deputise for lead nurse in managing the anticoagulation service, in her absence, including audit and follow up of patients not attending
  • to oversee the infection prevention and control guidelines and policies for the practice: participating in local meetings and training and disseminating information back to the practice. This includes advising partners and staff with regard to infection control matters and participating in the audit and the development of policies, assisting the lead nurse in implementing evidence based practice.
