Job summary

We are looking for highly motivated Enhanced Nurse Practitioner to join our dynamic and friendly team to work in state of the art GP Led Health Centre, rated "Outstanding " by CQC, in Pinner which is open 7 days a week. We are an innovative and forward thinking EMIS practice committed to delivering the highest standard of patient care, and proud to have a well-established and experienced nursing and multidisciplinary team.

Main duties of the job

Your role will require you to participate in patient care through knowledge gained from your training and previous work experience. As an Enhanced Nurse Practitioner, you will provide care for patients from initial history taking, clinical assessment, diagnosis, treatment.

  • To work within our experienced clinical team to help support patients with chronic long term conditions and patients living with frailty
  • Provide professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence
  • To contribute to and attend education and training to other nurses, multidisciplinary team members and students
  • To consult patients independently, seeking clinical advice and support where required, for minor ailments and long term conditions. Independent prescribing would be desirable but not at all essential

Job responsibilities

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

Job Responsibilities:

Supporting patients living with frailty and long term conditions

Working with practice GPs and practice frailty lead GP to support patients living with frailty and/or long term conditions in the community and care home

Assess and/or case manage patients with frailty in the community with support of the regular GP and the frailty lead GP, through telephone, practice-based and home visit assessments where necessary.

Work with and refer to other community providers to support patients living with frailty, where appropriate

Diagnosing and treating patients presenting with minor illness

  • Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
  • Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary

Personalised Case Managing and assessment of at risk patients

Supported by the patients named GP, work in creating, reviewing and implementation of personalised care plans for patients identified as vulnerable or at risk of unplanned admission to hospital. This may involve visiting patients in their own homes or in nursing /residential care homes to carry out the assessment and create the care plan

Liaising and working with professionals from community services, community MDTs and specialists to ensure they are delivering effective care to patients requiring this as required

The following skills would be desirable:

Able to perform simple clinical examinations

  • Urinalysis, other baseline observational assessments
  • Dressings
  • ECG
  • Phlebotomy (where necessary only)
  • Routine injections
  • Minor injuries
  • Routine immunisations and vaccinations
  • Other routine practice nursing skills

Pathological specimens and investigatory procedures if required

  • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures if required on a home visit or where necessary at the practice

Training and personal development

  • Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
  • Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.
  • Develop and maintain a Personal Learning Plan
