Job summary

Sett Valley Medical Centre is looking for an enthusiastic and experienced Advanced Clinical Practitioner to work 2 days per week, Tuesday and Wednesday, with an independent prescribing qualification to perform as an autonomous practitioner independently and as part of a multidisciplinary team to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions.

Main duties of the job

The post is intended to support our existing clinical team and is a permanent position. As an Advanced Clinical Practitioner the successful candidate will work to provide excellent care for our patients, and will be able to independently prescribe, assess, diagnose and treat patients at an ACP level.

The role will include a mixture of face-to-face, telephone and home visit appointments to meet the needs of our patients.

Sett Valley Medical Centre is a semi-rural 3 partner practice caring for approx. 10,500 patients, situated in the High Peak of Derbyshire, on the edge of the Peak District National Park, close to Stockport with excellent road and rail links to Manchester.

We are based in a purpose-built surgery, with an outstanding CQC grading, and our ethos is to deliver high-quality medical care through our comprehensive, highly skilled team members.

We have a strong and active patient participation group, and good community health care links. The surgery is paper light and uses EMIS Web.

Job responsibilities

The following are the core responsibilities of the Advanced Clinical Practitioner (ACP). There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Key Duties & Responsibilities

Clinical

Provide direct access to patients seeking primary health care; face-to-face, on home visits and on the telephone;

Work within the policies and procedures of the practice

Assess, diagnose, plan, implement and evaluate interventions/ treatments, and care for patients presenting with an undifferentiated diagnosis, referring to secondary or community care as appropriate;

Assess, diagnose, plan, implement and evaluate interventions/ treatments for patients with complex needs, and maintain knowledge of chronic disease processes and their management;

Proactively identify, diagnose and manage treatment for patients at risk of developing long term conditions;

Prescribe and review medication for therapeutic effectiveness appropriate to the patients needs, in accordance with national and practice protocols and within scope of practice;

Work with patients and pharmacists to support concordance to treatment, and provide information and advice on prescribed or over-the-counter medication, side effects and interactions;

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations;

Support patients to adopt health promotion strategies and self-care, referring as appropriate;

Participate in immunisation programmes;

Support patients in managing long term conditions and help the Practice to meet QOF targets

Prioritise and develop the management of housebound patients, managing complex needs and multipathology, and liaising with GPs, voluntary sector, district nurses or community matron as appropriate;

Develop models of good practice from internal and external sources to improve clinical care within the practice team, and hence help in reducing admission, prescribing and duplication of work
