Job summary

We have an exciting opportunity for a Nursing Associate to join our multi-professional Primary Care Team under the supervision of the Nurse Leads and Clinical Directors.

The purpose of this role will be to assist the clinical team in the provision and delivery of prescribed programmes of patient care and Long-Term Condition Management, strictly in accordance with local Primary Care Network workstreams and protocols.

This role is part of the extended hours provision and will include Saturdays and evenings on a rota basis with a day off in the week.

Main duties of the job

  • Health promotion and screening.
  • Undertake base line observations including BP, pulse, respirations, weight, height, and glucose monitoring.
  • Requesting pathology tests, for example urine culture, swabs.
  • Venepuncture, ECGs, and routine observations.
  • Performing approved diagnostics e.g., 24-hour BP/ECG monitoring, FeNo Testing, (additional training will be provided for this)
  • Wound care and removal of sutures/clips.
  • Smear Tests
  • Childhood, Adult and Travel Immunisations
  • Annual reviews and management of Long-Term Conditions including asthma, COPD, hypertension, and diabetes (additional, accredited training will be provided for this).
  • Supporting Personalised Care Planning (additional training will be provided)
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Home visits
  • Effectively manage own time, workload, and resources.
  • Report any safeguarding issues concerning children or vulnerable adults.
  • Work effectively with individuals in other agencies to meet patients needs.
  • Any other delegated duties appropriate to the post.

PCN 1W is in the West of Barnet with 4 GP practices over 5 sites and a diverse population, which is predominantly urban.

The practices are based in some of the most deprived areas of Barnet.

Job responsibilities

  • Maintain effective communication with staff members, patients, carers, and other members of the primary care team.
  • Refer patients directly to other services/agencies as appropriate, to promote integrated and seamless care.
  • Maintain accurate and complete patient records of patient consultations and enter on to computer systems using agreed templates and read codes.
  • Communicate clearly and appropriately, adopting the principles of safe practice and assessment of competence.

  • Provide a high standard of professional conduct and nursing care at all times in adherence with the

NMC Code of Conduct, maintain own registration with the NMC.

  • Maintain own continuous professional development, undertake annual appraisal, and ensure PDP kept up to date, to meet requirements of revalidation, including mandatory updates.
  • Ability to obtain and document informed consent (either verbal or written).
  • Ensure infection control guidelines are maintained.
  • Ability to monitor and manage maintenance of stock and equipment to include refrigeration protocols and emergency equipment.
  • Confidentially of information gained at work must be preserved at all times.
  • Work within your clinical remit and ensure patients are directed towards the most appropriate member of the clinical team.
  • Able to utilise a variety of IT packages, such as EMIS Web, teams, etc.
