An exciting opportunity has arisen for 2 Care Navigators / Receptionists to join our busy team, working at both our Lofthouse Site and The Manse, our Rothwell Site.
The practice working day is between 07:45 and 18:30, the successful applicant will be willing to work flexibly when needed.
Please see details of the available vacancies below (Monday - Friday):
- JOB A - 25 hours a week over a 5 day period- 5 hours a day on a rota basis (your shifts would be either 07:45 - 12:45 or 13:30 - 18:30, depending on the need of the business)
- JOB B - 30 hours a week over a 5 day period - 6 hours a day from 12:30 - 18:30

Main duties of the job
Care Navigators are the first line of contact with our patients and visitors. This is a varied role that includes face to face and telephone contact with patients as well as administrative duties.
You will work as part of a team of
Care Navigators, maintaining a high quality and efficient service for the patients,
Doctors and Nurses.
Reception experience/customer service is desirable, but training will be given to the successful candidate.

Our surgery provides care to over 11,000 patients across 2 sites: Lofthouse Surgery and The Manse Surgery. We have 8 Doctors, 3 qualified Nurses and 3 Health Care Assistants who work at the practice, along with our Care Navigation & Administration team.
We are a member of the NHS Leeds Integrated Care Board and we are also a member of the Leeds 25/26 Primary Care Network.

- Provide general assistance to the Practice team
and project a positive and friendly image to patients and other visitors,
either in person or via the telephone.

to appropriate NHS services or appropriate Health Professional to meet their
needs.
- In addition, the post holder will also support
the multi-disciplinary team, providing administrative support as required, in
line with the needs of the organisation.
