Wem & Prees Medical Practice is seeking an experienced and motivated Clinical Coder to join our friendly and busy primary care team.
This is a key administrative role responsible for transforming clinical information from patient records, discharge summaries, and correspondence into standardised clinical codes (e.g., SNOMED CT) within the practice system (EMIS Web). You will triage incoming documents, summarise patient notes, and ensure accurate, consistent data entry to support patient care, audit, and funding requirements.
We are looking for a reliable individual with strong attention to detail, excellent organisational skills, and the ability to work both independently and as part of a multidisciplinary team.

Main duties of the job
Review and triage incoming clinical correspondence (discharge summaries, clinic letters, reports)
Extract relevant clinical information and assign appropriate SNOMED/read codes within EMIS Web
Identify items requiring GP review or action
Summarise historical paper and electronic patient records into the clinical system
Ensure patient records are accurate, complete, and coded consistently
Process all correspondence (digital and paper) efficiently and accurately
Support workflow optimisation within the practice
Assist clinical staff with administrative tasks as required
Maintain strict confidentiality in line with GDPR and practice policies
