Job summary
We are looking for a pharmacy technician, who has a passion to deliver good quality care for frail older people living in a care home or would like a new challenge to apply existing knowledge and skills.
You would be joining our multi-disciplinary care home team, which comprises of advanced clinical practitioners from nursing, pharmacy and paramedic background as well as a registered nurse and care coordinator. The wider PartnersHealth team has a well-established, friendly team of pharmacists and pharmacy technicians at Rushcliffe PCN to provide the wider pharmacy support.
Main duties of the job
The medications management for residents living in a care home can be fragmented, and ever changing. As the pharmacy technician in the team, you will work with the care home staff to ensure medications are aligned, non-complex structured medication reviews are completed. You will support the ACP clinicians when they are undertaking resident reviews in the care homes, ensure new residents and residents post discharge have their medicines reconciled on the clinical system and medications ordering is as seamless as possible.
You will also be a point of contact to provide advice and guidance within skill, knowledge and capabilities competencies to the care home staff and the care home team clinicians and care coordinator.
You will receive clinical supervision and will be encouraged to extend knowledge and confidence in your new role.
