Job summary

An exciting opportunity has arisen for a Pharmacy Technician to join our growing pharmacy team. We are looking for collaborative, motivated and forward-thinking technicians. The role will involve supporting medicines management by working within the general practice setting, with the support of our Clinical MDT including Senior Clinical Pharmacists and GPs. You will provide specialist medicines management advice to patients and other healthcare professionals, to promote the safe, effective and high quality, cost effective use of medicines in all prescribing decisions. The role will also focus on improving the repeat prescribing process, relieving clinical workload pressures.

The PCN recognises that pharmacy technicians will play an important role, complementing our existing clinical pharmacist team, as well as working alongside community pharmacies, GPs and other members of the PCN multi-disciplinary team.

Main duties of the job

Pharmacy technicians core role responsibilities will cover clinical, and technical and administrative categories. The following sets out the key role responsibilities for pharmacy technicians:

Clinical

  • Undertaking patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

  • Undertake Long Term Conditions reviews and ensure appropriate compliance with national and local guidelines.

  • To provide telephone advice to patients with LTCs on specific medicines management issues

  • Provide health promotion where appropriate, including flu vaccinations.

  • Carrying out medicines optimisation tasks including effective medicine administration (e.g., checking inhaler technique), supporting medication reviews, medicine awareness and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

  • As determined by the PCN, supporting medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacists, and referring to the pharmacist for structured medication reviews.

  • Providing specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

  • Taking a central role in the clinical aspects of shared care protocols and liaising with practice pharmacists and GPs for more complex patients.

  • Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.

  • To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

  • To participate in high risk drug monitoring and medication audits.

  • To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing.

  • To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

  • To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

  • To advise on the sourcing and safe management of medicines as appropriate.

  • Developing relationships with other pharmacy technicians, pharmacists, and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health.

  • Supervising practice care navigation teams with prescription request reallocation equitably across all GPs

  • The role will also require pharmacy technicians to support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
