Job summary

We are seeking a motivated practice nurse to join our small friendly team. We offer an opportunity where no 2 days are the same and where you will have the opportunity to help shape and develop the practice through the oncoming changes for the healthcare system.

Main duties of the job

Support QOF achievement and clinical audits using SystmOne. Carry out various immunisations for children & adults, perform cervical screening, give travel health advice. Deliver health promotion and screening, smoking, alcohol, weight and more. Perform Long Term Condition reviews. Be part of emergency response preparation e.g. anaphylaxis, asthma attacks. Lead on elements of infection control, governance and patient safety.

About us

We are a small friendly practice with 3600 patients located in the Hallam and South locality of Sheffield. We are part of the Township 2 primary care network which consists of 5 practices.

Our present clinical team comprises of 1 GP Partner, 4 Salaried GP's, 1 Practice Nurse and a HCA. These are accompanied by a group of 5 Receptionists a Practice Manager and an assistant Practice Manager.

We also benefit from an excellent group of ARRS staff which support and advance the services we provide.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager

b. To support in the publication of the IPC annual statement in conjunction with the practice manager

c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

d. To develop, implement and embed health promotion and wellbeing programs

e. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

f. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

g. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

h. As required, to diagnose and manage acute and chronic conditions

i. Provide wound care (ulcer/Doppler, etc.) to patients

j. To review patient medications to enhance compliance

k. To co-ordinate the provision of travel medicine services

l. To request pathology services and process pathology results as required

m. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

n. To maintain accurate clinical records in conjunction with extant legislation

o. To ensure SNOMED CT codes are used effectively.

p. To ensure the appropriate maintenance and use of chronic disease registers

q. To develop, implement, embed and participate in well woman clinics

r. To develop, implement, embed and participate in well man clinics

s. To ensure compliance with the practice chaperone policy

v. To recognise, assess and refer patients presenting with mental health needs
