Job summary
To
provide nursing treatments and chronic disease management to patients in
participation with health care professionals and service users, working in
collaboration with other members of the team or independently to agreed
protocols, delivering quality patient services.
You
must be able to apply safe clinical decision making and expert care, whilst
remaining within your professional boundary.
Use your clinical skills to provide education to service users,
promoting self-care and empower them to make informed choices about their care.
You
will also provide management of the nursing team with responsibility for the
teams rota/clinics, return to work reviews, appraisals, time sheets, annual
leave requests and be the first contact for HR issues and determining whether
it needs to be escalated to the Senior Manager.

Probus is our main surgery, with branch sites at The Merlin Centre and Tregony. following are the core responsibilities of a Practice Nurse. There may be occasions when there is a
requirement to carry out other tasks; this will be dependent on factors such as
workload, staffing levels and direction of the Clinical Team or Senior Manager.

Independently
assess, diagnose, treat, refer, signpost and discharge patients and service
users as necessary and in accordance with clinical based evidence and NICE
guidelines

Undertake
chronic disease reviews, including diabetes, COPD, asthma, heart failure,
learning disabilities, etc.

telephone triage

Undertake the collection of pathology specimens, including blood
samples, swabs, etc.

Undertake simple wound and complex dressings, inclusive of leg
ulcers

Undertake contraception/pill checks and support our woman health
clinics

Perform investigatory procedures as requested by colleagues

Use appropriate infection control procedures
