Job responsibilities
The following are the core responsibilities of a Practice
Nurse. There may be occasions when there is a requirement to carry out
other tasks; this will be dependent on factors such as workload, staffing
levels and direction of the Clinical Team or Senior Manager:
Independently assess, diagnose, treat, refer, signpost and discharge
patients and service users as necessary and in accordance with clinical based
evidence and NICE guidelines
Undertake chronic disease reviews, including diabetes, COPD, asthma, heart
failure, learning disabilities, etc.
Undertake telephone triage
Undertake the collection of pathology specimens, including
blood samples, swabs, etc.
Undertake simple wound and complex dressings, inclusive of
leg ulcers
Undertake smears
Undertake contraception/pill checks and support our woman
health clinics
Perform investigatory procedures as requested by colleagues
Use appropriate infection control procedures
Be aware of safe storage of equipment, and drugs
Ensure clinical treatment is safe, effective and conducted
within the boundaries of competence, and to acknowledge limitations
Ensure personal and professional clinical standards are
maintained
Adapt styles of communication in order to empower
self-management and health education for patients
Participate in vaccination programmes, vaccinating patients
in the Practice, and/or on home visits
Formally and informally, impart knowledge and skills to
colleagues, promoting peer review and best practice within the working
environment
Communicate at all levels within the team, ensuring an
effective service is delivered
Work within Practice policies and procedures
Enhance own performance through continuous professional
development, imparting own knowledge and behaviours to meet the needs of the
service
Contribute positively to the effectiveness and efficiency of
the team, and work colleagues
Establish and maintain good liaison with other Practices, and
the wider system, including secondary care
Support the clinical team with safeguarding matters, in
accordance with local and national policies and refer safeguarding concerns,
using established safeguarding referral pathways
Delegate tasks clearly and appropriately, adopting the
principals of safe practice and assessment of competence of those colleagues
taking on the delegated duty
Ensure accurate and legible notes, of all consultations and
treatment, are recorded in the patients notes
Ensure the clinical system is kept up to date with accurate
coding, using templates where appropriate
Ensure the appropriate items of service claims are made
accurately, reporting issues to the relevant person
Collect and maintain statistical information, required for
regular and ad hoc reports and audits
Attend and participate in Practice meetings, as required
Undertake any other additional duties appropriate to the post
as requested by the Partners or Managers
