Job summary
The 'Work Well' programme aims to support people with MSK conditions to improve function, reduce pain, prevent deterioration and enhance overall wellbeing.
The service focuses on early intervention and supports self-management, reducing reliance on primary and secondary care whilst promoting holistic lifestyle changes and making every contact count.
This role will be working 1 day per month
Main duties of the job
Running 1 session per month in the community to support patients who are in employment but have had 2 or more fit notes issued to get back to work by pain and stress management.
You will be working in a multi-disciplinary team to support patients in a community location, GP's will refer to Work Well coaches which will then forward patients to the team to support
