Solent NHS Trust is looking for an enthusiastic and capable individual to join our Community Nursing team in Southampton as a Healthcare Support Worker within Case Management in the Southampton Community Nursing Service. Primary base to Bitterne Health Centre.

The successful candidate will work as part of a community nursing team supporting the district nurses and case management teams in nursing assessment, care and treatment of both adults and older people living at home who have long-term or complex physical health needs preventing them from accessing clinic based care. Candidates will be dynamic; able to work independently; have effective time management skills and will work well autonomously and as part of the team.

Previous community care experience is desirable. The service operates between the hours of 8.30am to 4.30pm.

Possession of an NVQ Level 3 (Diploma) or NVQ Level 4 (Diploma) or equivalent in care is preferred and you will be required to drive and have access to a vehicle for work. Regular support, supervision and training will be given.

Access to a vehicle and possession of a full driving license is essential.
