This is an exciting time to join our team as the service is currently undergoing a programme of transformational change. The post holder will participate in the delivery of skilled nursing care to patients within their own home, including care at the end of life, wound management, medicines management and as appropriate, assisting the patient to develop self-management skills. The community team provides a range of services to prevent unnecessary admission to hospital, to support early discharge home and to provide rehabilitation programs at home with the ultimate aim for most patients of independence and self-management.
