First Community is an outstanding provider of community services, within a developing forum of Primary Care Networks, an emerging Integrated Care Partnership, along with recently moving into the Surrey Heartlands Integrated Care System. It is therefore a time of transition into new systems, with evolving work streams looking at place-based and population based care.

This is an exciting time to be working within community services, with change occurring at pace, and the Transformation and Project Management Lead role is pivotal in supporting the operational leadership within the organisation with this change. The post is required to seek out concepts for change from outside of the organisation, benchmarking our service delivery, interpreting national and local guidance and strategic direction of travel, and translating that for the organisation to steer our transformation agenda.

You will be joining a friendly, professional and clinically-led social enterprise with an outstanding reputation with system partners. You will utilise and apply a range of service redesign and change management techniques alongside effective programme management skills to deliver our organisational and system wide objectives. You will be responsible for overseeing the ongoing development and maintenance of a suite of assurance processes and documentation, including project plans, to ensure that change is delivered effectively within a robust governance framework.

Recruitment and Retention Premium

Recruitment and Retention Premium applies to the following posts:

Successful applicants within the staff groups above will receive -

  • £1000 upon commencement (payable in your first months’ salary) then;

  • £1000 after 6 months service subject to satisfactory completion of your probationary period.
