Leicestershire Partnership NHS Trust provides a range of community health services for both physical and mental health conditions, as well as a range of specialist services supporting people of all ages including family and young people services, mental health, learning disabilities and physical health community and inpatient services.

We deliver services to the people of Leicester, Leicestershire and Rutland. Our 5500 staff serve a population of one million from over 100 sites across the region.


An exciting opportunity has arisen for two passionate, forward thinking Band 6 RMN's within the Integrated Care Mental Health Service Full-Time 37.5hrs per week covering Leicester City. Please note 30 hours would be considered for the right candidate.

Are you motivated, organised and above all have the passion and drive to have a positive impact on the health and well-being of patients with a Mental Health condition? Integrated Care Mental Health, based at Springfield Road Health Centre, provides a short term service (up to 12 weeks) within Leicester City.

Our patients are mainly aged 65 years and over and are often frail or have long term physical health conditions in addition to concerns about their mental health. The Service also sees those Patients aged 18-64 who have a long term physical health condition that is directly impacting negatively on their mental health. The Service currently takes referrals from 24 GP surgeries within Leicester City with plans to extend further in the coming months.

As a result of our patient group having co-existing physical and mental health conditions, our work is varied and at times challenging, but above all extremely rewarding.

You will be working as part of a small but dedicated team made up of one Band 7, four Band 6 Mental Health Nurses, one Occupational Therapist and two Health Care Support Workers. The Team works closely with GPs, Care Navigators, Specialist Physical Health Nurses and District Nurses. The Team attends regular MDT meetings with GP surgeries and also has close links with Secondary Mental Health.

You will be responsible for completing comprehensive initial mental health assessments and using your clinical reasoning skills to ensure that the most appropriate Integrated Care Clinician is involved to provide the on-going treatment and support. You will also be responsible for referring Patients on to Secondary Mental Health in a timely manner for those who require further assessment or longer term support.
