The Enhanced Health in Care Home (EHCH) is an exciting new service currently being developed and refined within Portsmouth.

We are looking for a compassionate, dynamic, and experienced Band 5 Nurse RGN to join the Enhanced Health in Care Home Team to help us to continue to provide excellent holistic, physical and mental health care within Portsmouth City. This is an exciting time and, in recognition of the important role community nurses play in supporting residents to remain within their care home, investment has been made into the development of a number of new posts.

This is an excellent opportunity to work as part of Multi-disciplinary Team (MDT) that consists of a dedicated GP, Pharmacist, and Occupational Therapist within the care homes and pro-actively work alongside the care home staff. Using your autonomous advanced clinical skills, you will case manage and support care home staff to develop robust and clear care planning for older people with complex needs, including ‘rest of life’ planning, end of life planning and management strategies for crisis intervention.

The emphasis of care is to keep people well and to prevent unnecessary hospital admissions.

There is a strong educational element to this role and the post holder will assist in identifying training needs and gaps within the care homes and work in partnership with the wider MDT and care home to develop and deliver training packages to meet these needs.

Your excellent clinical, interpersonal and team skills will enable you to work collaboratively across community, primary, secondary and social care settings.
