We are looking for a motivated, enthusiastic, prescribing triage nurse to enhance our current team. This is an exciting time to Join ELMS as you will be helping to roll our new acute patient assessment service ( APAS). The service will evolve over time and you will help to shape the direction of the service. You will prioritise the calls and direct patients or health professionals to the most appropriate service, trying to avoid inappropriate hospital admissions, keeping patients at home where possible. Prescribing is essential as the consultation may result in a prescription needing to be issued. You will also be involved in care planning and liaising with GP practices to maintain and obtain these for patients , working closely with the integrated neighbourhood teams and care co coordinators.

On behalf of the service you will carry out complex clinical triage in a sensitive manner providing assessment, advice and information , whilst utilising professional judgement and referring to other agencies/ services where appropriate.

Hours of work will be Monday – Friday initially, flexible hours including late shifts to meet the needs of the service, hours offered are for 30hours a week , and office based. You will be based at East Lancs medical services headquarters, Accrington road, Blackburn.
