This post is for an experienced pharmacist to join our expanding and dynamic Medicines Management team following an internal promotion. It would be suitable for someone wishing to gain further experience in a clinical role or for someone returning to work after a career break. Although the position is full-time, applicants wanting a part-time position will be considered.

The post will involve working with specialist pharmacy technicians to deliver a high quality and effective medicines management service to our patients in adult bedded units which are managed by Brighton and Hove City Council. You will also be working alongside GPs and staff employed by Brighton and Hove City Council to deliver excellent and safe patient-centred care.

You will also work alongside the senior clinical pharmacist and members of the multi-disciplinary team in Responsive Services which includes: nurses, occupational therapists, physiotherapists, community reablement assistants, advanced practitioners, social workers and mental health liaison nurses. You will assist the senior clinical pharmacist with domiciliary visits to patients, helping to undertake clinical pharmacy medication reviews of patients’ medicines to improve patient outcomes focusing on quality and safety. You will maintain effective working relationships, liaising with GP practices, community pharmacies and social services as required to help deliver medicines optimisation for patients.

To be considered for this exciting opportunity you will have been working as a clinical pharmacist directly with patients with complex health needs and with a range of healthcare professionals. You will also have excellent communication and interpersonal skills and proven team working abilities, be resourceful, able to manage your time effectively and prioritise your own workload.

This post will be based at Brighton General Hospital and you will be expected to travel across the geographical area covered by Brighton and Hove CCG. You will also be expected to periodically travel to other areas covered by Sussex Community NHS Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
