Primary Healthcare Darlington is a GP Federation which supports GP practices in Darlington to provide the best possible healthcare services.

We have an excellent opportunity available for enthusiastic and motived people to join our team. Working within this team you would have the opportunity to work alongside GP practice staff supporting patients over the age of 65 with moderate frailty.

This is a relatively new service and as an experienced member of staff you would have the opportunity to help shape and develop the service as well as providing leadership to a team of 3 well-being facilitators

The post is full-time Monday to Friday 9.00am – 5.00pm and is fixed term for a period of 2 years..

You will be based at one or more of the 11 GP practices within Darlington with the requirement to travel throughout Darlington.
