Dynamic motivated senior clinical pharmacists required in Hounslow Primary Care Networks (PCN) – Chiswick, Brentford and Isleworth (Brentworth), Feltham PCNs

Senior General Practice Clinical Pharmacist - Band 8a

As part of the NHS Long Term plan, the NHSE "Clinical Pharmacists in GP practice” initiative is an exciting element of the integrated care transformation agenda, looking at new ways of working and new models of patient-centred care. The integrated care programme as a system will develop closer links between secondary care, community trusts, community pharmacists, CCG and primary care pharmacists and we will be looking at opportunities to work across Hounslow to the benefit of our residents.

We are seeking senior clinical independent prescribing pharmacists to develop medicines optimisation services within GP practices. This is an exciting opportunity for senior clinical pharmacists to work as an integral part of Hounslow GP Primary Care Networks (PCN)

The three Primary Care Networks looking to employ Senior Clinical Pharmacists are:

  1. Chiswick Primary Care Network
  2. Brentworth (Brentford and Isleworth) Primary Care Network
  3. Feltham Primary Care Network

Hounslow & Richmond Community Healthcare NHS Trust will host the role. However, the role is entirely practice based and the successful applicants will have the opportunity to embed the role of the senior clinical pharmacist within the Primary Care Networks.

The senior pharmacist will take responsibility for areas of chronic disease management within the practices, undertake clinical medication reviews, manage patients with complex poly-pharmacy, as well as developing and leading other clinical pharmacists within the network. They will also provide primary support to general practice staff with regards to prescription, medication queries, potentially developing a repeat prescription hub model and targeting care homes. They will deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of the patient.

This exciting position requires innovative and visionary pharmacists. The successful applicants will need to be self-motivated and able to work on their own, whilst also integrating into a number of well-established General Practices. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the NHS.
