Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire and Sefton area. The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites.

Summary of role:

If you are a NMC registered nurse who is interested in acute, client centred care within the community, we are looking for an enthusiastic and motivated registered nurse to join the Blackburn with Darwen Intensive Home Support Service (IHSS) (part of the Lancashire Care Foundation Trust - Adult and Community network). The team is an integrated multi-disciplinary team of Occupational Therapists, Physiotherapists, Nurses, Assistant Practitioners, Complex Case Managers and Support Workers.

This nursing role involves assessing patients within their own home environment to reduce avoidable admissions and support people within the community at times of crisis due to deterioration in health.The IHSS service operates 8.00am - 8.00pm 365 days a year.

You will need excellent communication skills and have experience of working as part of a team. The role will involve delivering excellent, evidence based nursing care. The role will involve a holistic approach to emphasising the elements of rapid response, intravenous therapy, Complex Case management, recuperative and rehabilitative care.

This post is subject to an enhanced DBS Disclosure and the successful applicant will be required to pay for their DBS check.
