The Wandsworth Maximising Independence is a dual pathway for patients within Wandsworth who require rehabilitation and/ or reablement. The service is part of the Wandsworth Community Health Service, and prides itself with its integrated working. We work alongside our nursing colleagues, GPs and specialist community services to provide excellent locality based care to patients within Wandsworth. We work closely with our partners in local acute care providers, social care and third sector to better meet complex patients’ needs.

The team comprises of a large multidisciplinary team; physiotherapists, occupational therapists, speech and language therapists, rehabilitation assistants and support workers. It provides a urgent and routine pathway to adults over 16 years of age, working with them to regain their independence following illness or injury including falls, preventing avoidable attendance or readmission to hospital, supporting them to manage long term conditions and promoting health and wellbeing.
