Proactive Care, part of the Rural Local Community Network, is a major initiative between health and social care agencies. The aim is to integrate local health, care, council, voluntary services and communities, with a focus on the needs, priorities and assets of the people who live within their network.

An exciting and rare opportunity has arisen for a Proactive Care Coordinator in the Rural North Chichester Primary Care Network. The role involves working very closely with the multidisciplinary team (MDT) and a caseload of patients, looking at the patients' needs and preventing unnecessary hospital admissions. The Proactive Care Coordinator role is pivotal in ensuring all our patients receive the best possible care and service.

This role will support the clinical and social care professionals in coordinating all key activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred.

The successful candidate will need to have a good understanding of health and social care services, be an excellent communicator, and be able to establish efficient monitoring and quality assurance systems. NVQ Level 3 or equivalent is required.

You will need to be a car driver and have access to a car to travel between sites.
