About Barnet Federated GPs

Barnet Federated GPs CIC is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet. The Federation is building a large team who will work as one in developing Primary Care Services in Barnet and will support Primary Care Networks.


The Federation was formed in 2015 and aims to improve the quality of health in Barnet through sustainable primary care. Its current membership consists of all 52 primary care practices within the borough. Although we are a private company, we hold a strong affiliation to the NHS, its constitution and guiding principles.


The Role

Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

Link workers also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.

It’s an exciting time to join us, as we are looking for enthusiastic and motivated Link Workers to come and work within the multi-disciplinary teams across our practices, establishing our new social prescribing link worker service and delivering personalised care to our patient population. You will work as a team member of the pan Barnet Social Prescribing Link worker service, sharing best practice and knowledge for the benefit of patients.

You will be working in partnership with local voluntary and community services and agencies, ensuring the best outcome for patients.


The Candidate

The successful candidates will be working within one of the Primary Care Networks in Barnet, supporting the General Practices and working with people who have long-term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing.

The successful candidate will be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges. You will have great interpersonal skills in supporting people, community groups and local organisations.
