Gloucestershire Health and Care NHS Foundation Trust was formed on Tuesday 1st October 2019, following the merger of 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust. The two Trusts joined together to provide integrated mental health, physical health and learning disability services to communities.


Community Nursing Assistant Practitioner

Band : 4

Salary: £21,089 - £ 23,761

Hours: 37.5 hours

Location: Cheltenham

An opportunity has arisen for a dynamic, Assistant Practitioner to join the Integrated Community Nursing Teams within the Cheltenham Locality.

As a Band 4, you will support the team in delivering the care of patients in their own homes, providing a range of clinical interventions.

You will need to be a flexible team player with sound clinical knowledge and excellent communication skills with the ability to work autonomously. You must be able to demonstrate professional knowledge and competence through ideally having or working towards a foundation degree.

The service by nature is busy and diverse in an ever changing environment of health and social care integration. The role requires day to day delivery of patient care in our ICTs alongside District Nurses, Community Nurses, therapists and social care staff.

We are a forward thinking trust, already working from electronic patient recording systems (SystmOne) and mobile working is embedded in our services.

We work between 8am and 8pm across 7 days a week.
