Welwyn and Hatfield Integrated Care team, based in Welwyn at the QVM hospital, are looking for a band 5 Physiotherapist to join our team. We work in a diverse community setting that offers a fantastic learning environment for you.

We are looking for someone with energy, enthusiasm and drive as well as excellent professional skills.

We work closely with our Nursing colleagues as well as GPs and other members of Adult Social Care Services, thereby ensuring you have every opportunity to apply your excellent interpersonal and communication skills.

The service is provided by Hertfordshire Community NHS Trust, Hertfordshire Partnership University Foundation Trust and Hertfordshire County Council’s Adult Care Services. The partnership organisations are working collaboratively across nursing, therapy, mental health and social care to deliver better care for our population

We provide input to a wide variety of patients within the Community Setting including those with neurological conditions as well as the elderly / frail population who have complex physical and social care needs. We also work closely with hospitals and primary care settings to support discharges and prevent the unnecessary admission of patients with acute or deteriorating conditions in the community providing rehabilitation where appropriate or advising on ongoing manual handling needs.

Supervision will be provided by Band 6 Therapists and there is clinical support for Physiotherapists through our Clinical Leads within the Trust.

This post also attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,017 and a maximum payment of £1,762 per annum (pro rata where applicable)
