Children's Continuing Care & Health Short Breaks Team are based at Hunslet Health Centre. Hannah House is our 6 bedded unit in Rothwell Leeds.
The teams are focused on enabling and supporting children and young people and their families to have as much care as possible outside of the hospital setting, and within their own communities. The majority of this support is carried out in the family home.
We are part of an innovative Community Children's Nursing Service, forward thinking and patient centred continually striving for excellence for our children and families. We promote cross team working and there is potential for you to have the opportunity to spend time in our Children's Community Nursing Team, Hannah House Short Breaks Unit and Inclusion Team within schools.
Informal visits welcomed.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.43 or Standard £30.43) from your salary. There will be no charge if you are enrolled in the DBS Update Service or a current employee of the Trust.

The successful candidate will be expected to work across the other children's community nursing teams when required.
