South Kirklees Enhanced Pathway is looking to recruit an enthusiastic and motivated individual to join our existing multi-disciplinary team as a Band 6 Mental Health Practitioner.

The South West Yorkshire Foundation Trust has recently transformed its Acute and Community services. As part of the transformation process the Enhanced Pathway provides services for people with complex mental health needs. The Enhanced Pathway provides a flexible assertive community treatment (FACT) model of care 5 days a week.

The service covers Monday to Friday 9am to 5pm. The post holder will be responsible for undertaking community assessments and delivering treatment interventions as well as triage of referrals and be responsible for assessments, care-planning and co-ordination of care delivery within a team approach.

The post holder will be expected to attend the daily morning meeting to feedback risk situation and to formulate care/risk assessment/management plans.

You should be forward thinking, proactive, and the ability to form effective professional working relationships with your team colleagues and service users is an essential requirement of the job as is effective time management skills.

Car driver status is essential to this post. A period of induction will be available for the successful candidate.
